Those eligible include those who are (1) 65 years of age and older, and/or (2) have severe congenital developmental or early-onset medical disorders. Those who complete the form will then be contacted at a later date with instructions to schedule an appointment for COVID-19 vaccination at the appropriate time. Persons working in K-12 schools returning to in-person learning on March 1st will be vaccinated in coordination with their school districts and should not complete this form.

COVID-19 vaccine is extremely limited, and LCGHD does not know how many doses will be received in the coming weeks. For this reason, completing a form does not guarantee a COVID-19 vaccine appointment. If you are in Phase 1B, and have previously contacted LCGHD or submitted an inquiry, you will need to submit information in this new form to be placed on the list. Please complete one submission per eligible person.

What is Motivate Lake County


The Lake County General Health District launched Motivate Lake County to encourage Lake County residents to take back control of their health. The initiative will promote existing health and wellness events/activities in Lake County and include new health fairs, wellness challenges, classes, presentations, support groups, and more. Motivate Lake County is a county-wide initiative designed to inspire, engage, and most importantly educate community members to become a healthier version of themselves. It will provide residents the tools and resources to achieve their unique health and wellness goals.
